As a result of your complaint, an investigation was conducted by this Department and based on the evidence presented; the following has resulted.An official warning letter is being sent to the facility. The facility has been cited with the following violations.Regulation 82.5(b) Willfully performed services not authorized in that facility failed to repair all collision damages to a 1994 Nissan Maxima exactly as authorized by xxxxxxxxxxxx.Regulation 82.5(c) Willfully failed to complete invoice as required in that the invoice provided to xxxxxxx on 05/30/03 does not note the correct status of the fan assembly actually installed in a 1994 Nissan Maxima.
